Is it possible to add combined conditions to the blacklist? (e.g. Email + IP address)

In this case, you can add a Decline rule containing two parameters (email and IP). However, if one of the two data points is already on the blacklist, the transaction will be automatically declined, since the blacklist is the strongest rule.
